To report a claim, please collect as much of the follwing information as necessary:

• Name and contact information for other parites to the accident and their insurance information

• Vehicle information for other parties (year, make, model, color, license plate and VIN)

• Contact information for any potential witnesses

• Name and phone number of investigating police department

• Detailed facts pertaining to the accident

• Notices of any injuries
